Play Group to Class XII
Dear Parents,
Please note that the annual medical checkup for all our students will be organized in the school premises in the month of November 2025.
We are distributing the Health Card to the students.
You are requested to fill up the personal details on the first and all the details on the last page along with your signature. The details should be filled in block letters with a black ball point pen.
Kindly note if a child is suffering from a specific ailment, a supporting medical document needs to be submitted.
The students must return the Health Card to their respective Class Teachers by Monday, 17th November 2025.
