Nursery – Class XII

Dear Parent,

The school is initiating the process of creating student records in UDISE+. As part of this statutory requirement, we are required to collect accurate and complete information from parents/guardians to create each student’s profile.

You are kindly requested to:

Fill in the attached UDISE+ Student Data Collection Form carefully and completely.

Write all entries clearly in BLOCK LETTERS.

Verify each detail before signing the declaration at the end of the form.

Ensure that no field is left incomplete, as this may affect the accuracy of the student’s profile in UDISE+.

Ensure mandatory attachments

A photocopy of Student’s Aadhar Card, attested by either parent.

In case of CWSN student, attach a photocopy of UDID Card, attested by either parent.

Incomplete or incorrect information may affect the accuracy of your ward’s official records. Please ensure that the completed form is submitted to your daughter’s Class Teacher by Friday, 28th August 2026.

Your cooperation in this process will help us maintain error‑free records and comply with statutory requirements.
