Play Group to Class XII
Dear Parents,
Please note that the annual medical check-up for all our students will be organized in the school premises from October to December 2026.
We are distributing the Health Card to the students.
You are requested to fill up the personal details on the first and all the details on the last page along with your signature. The details should be filled in block letters with a black ball point pen.
Kindly note if a child is suffering from a specific ailment, a supporting medical document needs to be submitted.
The students must return the Health Card to their respective Class Teachers by Wednesday, 28th October 2026.
