Nursery – Class XII

Dear Parents,

Please note that the annual medical check up for all our students will be organized in the school premises in the months of July and August.

With regard to this, you are requested to fill the first and last pages of the Health Card in block letters with a black ball point pen.  Please put your signature on the last page of the Health Card.

The students are expected to return the Health Card to their respective Class Teachers by Friday,  14^th July 2023.

Note – If a child is suffering from a specific ailment, a supporting medical document needs to be submitted.
