Nursery – Class IV

Dear Parents,

Kindly fill in the attached Proforma in block letters. Attach a recent PASSPORT SIZE photograph of your daughter in school uniform (with her name written at the back of the photograph) and return the same to your daughter’s class teacher by Monday, 4^th February 2019.

Kindly make sure that all the information is accurate, as this will be processed for the Identity Card.

Please note that KG students going to class I should provide the photograph in school uniform without the apron.
